At With Grace Concierge, I access provider notes through the patient portal, follow up directly with the doctor's office, and piece together the full picture โ then deliver it to you clearly. You're not replacing your relationship with your parent. You're adding a professional layer that love alone can't provide.

Three ways, used together. First, I call your loved one shortly after the appointment for their account of what happened. Second, I access their patient portal โ where providers post clinical notes and prescriptions within 24โ48 hours โ using login credentials provided during onboarding. Third, when needed, I call the provider's office directly as an authorized care coordinator using a signed release from your loved one.
On in-person visit days (Mondays and Tuesdays), I'm in the room โ which eliminates the information gap entirely. You'll receive a written family update within 48 hours of every appointment regardless of tier.
Doctors are managing hundreds of patients and are not responsible for keeping families updated after every visit. Unless your loved one has signed a specific release, HIPAA laws actually prevent providers from sharing information with family members without consent.
What With Grace Concierge does is obtain proper authorization from your loved one upfront โ so I can legally communicate with their providers, access their portal, and relay accurate information to you. We bridge a gap the medical system was never designed to fill on its own.
Your parent's doctor's job is to treat. My job is to make sure nothing gets lost between the treatment and your family.
No โ and that distinction matters. Home health aides provide hands-on personal care. Nurses provide clinical assessment and treatment. Complete Advocacy plan clients have access to an emergency coordination line. If something urgent arises โ a hospitalization, a sudden change in condition, a medication concern โ I respond immediately. I coordinate with providers, notify family contacts, help navigate next steps, and stay with the situation until it's resolved.
For Virtual and Hybrid plan clients, urgent situations are escalated within the same business day. You will never be left wondering who to call or what to do. That's the whole point of having us.
This is exactly the right question to ask. Every client signs a service agreement that includes a strict confidentiality clause. All client information is handled with HIPAA-standard care and discretion. I do not share, sell, or disclose any client information to any party outside of those explicitly authorized by your loved one.
